1. How it works — the mechanism
Omission bias rests on a deep moral intuition that most cultures encode in both law and ethics: doing harm is worse than allowing harm. Deliberately pushing someone off a bridge is murder; failing to save a drowning stranger is, at most, a failure of charity. This intuition is not entirely wrong — causation matters morally, and there are good reasons to hold people more accountable for what they actively cause than for what they merely fail to prevent. The problem is not the intuition itself but its application beyond contexts where it is appropriate.
Omission bias becomes a cognitive distortion when it leads people to prefer harmful inaction over equally or more beneficial action — when the asymmetric moral treatment of acts and omissions produces worse outcomes for everyone involved. A parent who refuses a life-saving vaccine because they fear the (low) risk of a side effect, while accepting the (higher) risk of the disease itself, is applying the act/omission asymmetry in a context where consequences — not causes — are the morally relevant consideration. Why omission feels less culpable — five mechanisms

Pushing someone into traffic directly causes their death; failing to warn them of an approaching car allows nature to proceed — even if the outcome is identical. The directness of causal responsibility feels morally relevant and produces asymmetric blame, even when consequentialists would argue only outcomes matter.
After a harmful action, the counterfactual "if I had done nothing, the harm wouldn't have occurred" is vivid and specific. After a harmful omission, the counterfactual "if I had acted, the harm might not have occurred" is speculative and less certain. The asymmetry in how clearly we can trace our causal role produces asymmetric guilt and blame — actions produce clear causal guilt; omissions produce uncertain causal guilt.
Actions express agency and intent — they put your fingerprints on an outcome in a way that omissions do not. For harms with emotional valence, this "fingerprint" creates a felt sense of contamination — "my hands are dirty" — that omissions don't trigger. The disgust-like response to causing harm through action is neurologically and psychologically more intense than the response to allowing equivalent harm, which feels more like bad luck than personal failure.
In most contexts, inaction is the social and normative default — doing nothing requires no justification, while acting requires a reason. When omission genuinely is the better choice
Not all inaction is biased inaction. Sometimes not acting is genuinely the right decision — in medical contexts, watchful waiting is often evidence-based; in investing, patience outperforms activity; in leadership, not intervening in a functioning team is correct. The goal is not to eliminate omission preferences but to ensure they are based on outcome analysis rather than on causal moral intuitions that don't apply.
Step-by-step design process for counteracting omission bias
- Identify where inaction is the current default and map its consequences. Audit the decisions in your context where "doing nothing" is the path of least resistance. For each, calculate the expected cost of that inaction — in health outcomes, financial returns, relationship quality, or organizational performance. If the cost of inaction is not visible and quantified, you cannot make it salient.
- Reframe the omission as an active choice with a specific causal chain. "Not deciding" is still deciding. Make this explicit: "Choosing not to vaccinate today means accepting a [specific] risk of [specific disease outcome]."
